How do bank fees, withholding taxes, and withdrawals affect the return calculation?
I would THINK that fees and withholding taxes should detract from returns — since the fees and taxes are costs of owning the investment, and should offset any gains. Is that the case?

This is a bit more complicated topic.
Withholding taxes does not affect return - depending on your country and tax arrangements you might or might not have withholding taxes, but it is just a pre-tax, and to be able to compare returns, we should not be considering withholding taxes.
